Chronic Condition Special Needs Plans

A practical guide to C-SNP eligibility, benefits and plan comparisons.

Reviewed October 2, 2026. This guide explains the plan type; it does not identify specific local plans or guarantee eligibility.

What is a C-SNP?

A C-SNP is a Medicare Advantage plan for people with designated serious chronic conditions. It combines Part A and Part B benefits with Part D drug coverage and care coordination. It is not Medicare Supplement insurance.

Who may qualify?

You must have Parts A and B, live in the service area and satisfy the plan’s medical eligibility criteria. Medicaid (Medi-Cal in California) is not required for a C-SNP; D-SNPs have different eligibility rules. The plan verifies your condition with your health care provider.

Examples include diabetes, chronic heart failure, certain cardiovascular or lung disorders, dialysis-dependent end-stage renal disease, and selected autoimmune conditions such as rheumatoid arthritis. Other categories include dementia, stroke, certain cancers and neurologic disorders. This is not a complete list.

A diagnosis does not qualify you for every C-SNP. Plans target particular conditions or combinations. Ask which diagnoses the specific plan accepts and what documentation it needs.

How could it help?

Care coordination connects your treatment needs, providers and medications. A care assessment and individualized plan can help organize your care. Drug lists and provider networks are designed around the population served. Some plans include dental, vision or hearing benefits; coverage and limits vary.

What costs and restrictions should I review?

You generally continue paying Part B premiums. Plan premiums, deductibles and copays vary. HMO or PPO network rules, referrals and prior authorization may apply. Review the Summary of Benefits, Evidence of Coverage, provider directory and formulary before choosing.

Can I enroll outside the fall enrollment season?

A qualifying chronic condition may provide a Special Enrollment Period to join an available C-SNP serving that condition. Medicare states that this opportunity ends once you join using it; it is not permission to switch plans repeatedly throughout the year. Other enrollment periods may apply. Confirm your eligibility and coverage start date before changing coverage.

Your comparison checklist

  • Does this exact plan accept my documented diagnosis?
  • Are my primary doctor, specialists and preferred hospital in the network?
  • Are all my medications covered, at what cost, and at which pharmacy?
  • What would I pay for specialist visits, hospital care and medical equipment?
  • What is the annual medical out-of-pocket limit, and which expenses do not count toward it?
  • How do I contact my care coordinator?
  • Do extra benefits have eligibility rules, limits or approved providers?
  • Will an existing treatment require a new referral or authorization?
